Dear Patients,                                                                                  
 
          I am writing to inform you that I will be closing the practice as of 12/31/25.  This decision was not made lightly, and I want to express my gratitude for the trust you have placed in us over the years.

          Please know that I am committed to ensuring a smooth transition for your ongoing healthcare needs.  I recommend you seek care from Teresa Slone, NP, who had been working with me for the past 8 years.  She will now be working at LifeSpring, (at Dr O’Brien’s office, located on 260 S Maple Street, Orleans 812.865.3350), effective January 5, 2026.  Dr. O’Brien is also available to take care of you. 

          Other clinicians in the area include those working with Comprehensive Health Care.  They have offices in Mitchell, Paoli, French Lick, Marengo, Bedford and other surrounding communities.

          If you are not acquainted with a clinician, you may use your insurance company’s provider directory or check with the hospital’s physician referral service.

          Once you have identified a clinician or practice, they can contact our office to obtain copies of your medical records.  You will need to sign a consent form to transfer records.  These records can be electronically sent to your new clinician.  If you prefer taking them yourself, there will be a nominal fee to print them.  If you choose to pick up a copy yourself, please request at the office at 812-865-3400 by Dec. 31, 2025.  After this, your records will be held by Orleans Medical Clinic for a minimum of 10 years before they are destroyed properly.  Please do not wait to do this until you need an appointment as Orleans Medical Clinic medical records department will be most accessible through March 2026.
